Care Coordination – saving employees’ time

A Care Coordinator follows each case from initial phone call through case closing. Knowing that they will only have to tell their story once helps employees develop a comfort level with the EAP and create an environment for successful case resolution. This personalized and high-touch process also ensures that employees get the right resources at the right time. Across BHS’ book of business, up to 65% of EAP cases are resolved within the scope of the program without using expensive mental health and substance abuse benefits from the employee’s health plan.
Linkage with other benefits

BHS Care Coordinators link clients requiring additional care to the necessary benefits (i.e. medical insurance, short and long term disability, managed behavioral health providers and/or community resources) to help ensure the most appropriate problem-resolution. This includes:
• Briefing the employee on programs and services covered by
their plans
• Notifying employees of publicly funded/ community based
options
• Briefing the employee on any financial obligations such as
co-pays
• Ensuring that all contact info provided by resources is valid,
current and accepting new clients
• Assisting employees schedule appointments with treatment
resources
• Transitioning necessary case information as appropriate
