How to Apply

You may also initiate the credentialing process by downloading the following application form:
Upon completion, please send your application packet to BHS’ Provider Relations Department at the address or fax number indicated on the application. Remember to include current copies of: your resume or curriculum vitae, clinical licensure/certification, and malpractice insurance certificate.
Upon receipt of your application, the Provider Relations Department will review your information and contact you directly to discuss the next steps
Credentialing Criteria:
Mental Health Professionals:
- Holds a minimum of a Masters degree in a behavioral health discipline.
- Licensed/Certified at highest level for discipline in the state in which you practice.
- Minimum of three years, full-time, postgraduate clinical experience in the direct provision of mental health and/or chemical dependency services.
- Professional liability insurance, minimum of $1 million per occurrence and $3 million aggregate.
- Availability of at least 20 hours per week in clinical practice.
- At least 2 years experience providing EAP services on at least a half-time basis (and/or experience in assessing for chemical dependency conditions).
Health & Wellness Providers (Nurses, Nutritionists, Fitness Professionals, Wellness Educators, Massage Therapists, Wellness Coaches, etc.):
- Licensed/Certified at highest level for discipline in the state in which you practice.
- Minimum of three years documented experience in your discipline.
- Understanding of holistic health and wellness.
- Professional liability insurance, minimum of $1 million per occurrence and $3 million aggregate.
- Comfortable providing services to a diverse population.
Critical Incident Stress Management (CISM) Providers:
- Licensed/Certified at highest level for discipline in the state in which you practice.
- Providers must hold Crisis Intervention Stress Management (CISM) certification.
- At least 1 year of experience providing CISM services.
- Understanding of holistic health and wellness.
- Professional liability insurance, minimum of $1 million per occurrence and $3 million aggregate.
- At least 5 years general clinical experience (can include pre and post graduate experience).
Affiliates Designated as Workplace Trainers:
- Holds a minimum of a Bachelors degree.
- Professional liability insurance, minimum of $1 million per occurrence and $3 million aggregate.
- At least 2 years of documented experience providing workplace trainings in areas such as Supervisory Trainings, Educational Trainings, Wellness Trainings, Employee Orientations, Organizational Development, etc.
- Familiarity with Microsoft PowerPoint for presentations.
- Comfortable providing trainings to a diverse workgroup (large or small).
Organizational Development:
- Holds a minimum of a Master's degree in a behavioral health or related discipline.
- Licensed/Certified at highest level for discipline in the state in which you practice.
- Minimum of three years, full-time, documented experience in the direct provision of mental health, organizational development, human resources or executive coaching services.
- Professional liability insurance, minimum of $1 million per occurrence and $3 million aggregate.
- Availability to assist with a project that could be ongoing for 1, 3, 6, 9, and 12 months respectively.
- Documented experience in providing services to a diverse population, especially to upper management, leadership and executive teams.
Executive Coaches:
- Holds a minimum of a Master's degree in a behavioral health or related discipline.
- Licensed/Certified at highest level for discipline in the state in which you practice.
- Minimum of three years, full-time, documented experience in the direct provision of mental health, organizational development, human resources or executive coaching services.
- Professional liability insurance, minimum of $1 million per occurrence and $3 million aggregate.
- Availability to assist with a project that could be ongoing for 1, 3, 6, 9, and 12 months respectively.
- Documented experience in providing services to a diverse population, especially to upper management, leadership and executive teams.
SAP (Substance Abuse Professionals):
- Holds a minimum of a Masters degree in a behavioral health discipline.
- Licensed/Certified as a SAP in the state in which you practice.
- Minimum of three years, full-time, postgraduate clinical experience in the direct provision of mental health
- and/or chemical dependency services.
- Professional liability insurance, minimum of $1 million per occurrence and $3 million aggregate.
- Availability of at least 10 hours per week for SAP services.
- At least 2 years experience providing SAP services on at least a half-time basis.
