Become a BHS Provider

How to Apply

If you would like to obtain more information about becoming a BHS network affiliate, please call our Provider Relations department at 1-800-942-6640 x 300 or send us an email message by clicking here. We will gladly contact you at our earliest opportunity, and can provide a secure login password to access additional information.

You may also initiate the credentialing process by downloading the following application form:

Upon completion, please send your application packet to BHS’ Provider Relations Department at the address or fax number indicated on the application. Remember to include current copies of: your resume or curriculum vitae, clinical licensure/certification, and malpractice insurance certificate.

Upon receipt of your application, the Provider Relations Department will review your information and contact you directly to discuss the next steps

 

Credentialing Criteria:

Mental Health Professionals:

  1. Holds a minimum of a Masters degree in a behavioral health discipline.
  2. Licensed/Certified at highest level for discipline in the state in which you practice.
  3. Minimum of three years, full-time, postgraduate clinical experience in the direct provision of mental health and/or chemical dependency services.
  4. Professional liability insurance, minimum of $1 million per occurrence and $3 million aggregate.
  5. Availability of at least 20 hours per week in clinical practice.
  6. At least 2 years experience providing EAP services on at least a half-time basis (and/or experience in assessing for chemical dependency conditions).

Health & Wellness Providers (Nurses, Nutritionists, Fitness Professionals, Wellness Educators, Massage Therapists, Wellness Coaches, etc.):

  1. Licensed/Certified at highest level for discipline in the state in which you practice.
  2. Minimum of three years documented experience in your discipline.
  3. Understanding of holistic health and wellness.
  4. Professional liability insurance, minimum of $1 million per occurrence and $3 million aggregate.
  5. Comfortable providing services to a diverse population.

Critical Incident Stress Management (CISM) Providers:

  1. Licensed/Certified at highest level for discipline in the state in which you practice.
  2. Providers must hold Crisis Intervention Stress Management (CISM) certification.
  3. At least 1 year of experience providing CISM services.
  4. Understanding of holistic health and wellness.
  5. Professional liability insurance, minimum of $1 million per occurrence and $3 million aggregate.
  6. At least 5 years general clinical experience (can include pre and post graduate experience).

Affiliates Designated as Workplace Trainers:

  1. Holds a minimum of a Bachelors degree.
  2. Professional liability insurance, minimum of $1 million per occurrence and $3 million aggregate.
  3. At least 2 years of documented experience providing workplace trainings in areas such as Supervisory Trainings, Educational Trainings, Wellness Trainings, Employee Orientations, Organizational Development, etc.
  4. Familiarity with Microsoft PowerPoint for presentations.
  5. Comfortable providing trainings to a diverse workgroup (large or small).

Organizational Development:

  1. Holds a minimum of a Master's degree in a behavioral health or related discipline.
  2. Licensed/Certified at highest level for discipline in the state in which you practice.
  3. Minimum of three years, full-time, documented experience in the direct provision of mental health, organizational development, human resources or executive coaching services.
  4. Professional liability insurance, minimum of $1 million per occurrence and $3 million aggregate.
  5. Availability to assist with a project that could be ongoing for 1, 3, 6, 9, and 12 months respectively.
  6. Documented experience in providing services to a diverse population, especially to upper management, leadership and executive teams.

Executive Coaches:

  1. Holds a minimum of a Master's degree in a behavioral health or related discipline.
  2. Licensed/Certified at highest level for discipline in the state in which you practice.
  3. Minimum of three years, full-time, documented experience in the direct provision of mental health, organizational development, human resources or executive coaching services.
  4. Professional liability insurance, minimum of $1 million per occurrence and $3 million aggregate.
  5. Availability to assist with a project that could be ongoing for 1, 3, 6, 9, and 12 months respectively.
  6. Documented experience in providing services to a diverse population, especially to upper management, leadership and executive teams.

SAP (Substance Abuse Professionals):

  1. Holds a minimum of a Masters degree in a behavioral health discipline.
  2. Licensed/Certified as a SAP in the state in which you practice.
  3. Minimum of three years, full-time, postgraduate clinical experience in the direct provision of mental health
  4. and/or chemical dependency services.
  5. Professional liability insurance, minimum of $1 million per occurrence and $3 million aggregate.
  6. Availability of at least 10 hours per week for SAP services.
  7. At least 2 years experience providing SAP services on at least a half-time basis.
